If your child plans to participate in the school athletic program for the school year 2008-2009 they will be required to submit a sports physical prior to the start of school. This physical must be on file at the school prior to your child starting practice of their sport. The Rockford Diocese also requires a copy of your child's health insurance card. The athletic program has other release forms to be submitted to them, but the school office requires the physical form and the insurance confirmation.

Students entering seventh and eighth grade who want to participate in sports will also need to submit a sports physical and proof of health insurance to the school office prior to the start of practice for their sport.
