Dear 5A Parents,

I would like to start an after school math help session for students who need extra help in math. Your child could come each week or only when extra help is needed.

In order for your child to participate, I will need the form signed and filled out by you. Your child will have to bring it to me by the date of the review class. The sessions will be on Tuesdays. On the rare dates that I am unavailable, I will notify you in advance.  

The sessions will start Tuesday, October 16^th. We will begin after school and end promptly at 3:00. I will walk your children down to the front door (or ET). 

I will be using a three strikes your out discipline policy for this activity.  Therefore, if your child needs to be corrected for misbehavior during a session, he/she will be given a strike.  Three strikes and they will not be allowed to attend the math review sessions for the rest of the school year.

 Math Review Permission
