Dear Parents,

Children entering Kindergarten (must be five years old by September 1st) must have a physical on file as required by Illinois State Law. By the start of the school year all students must have on file with the school a complete record of immunizations as required by the State of Illinois Department of Public Health. The only exemption accepted will be for medical reasons as certified by a M.D. or D. O. The doctor must complete the diabetes screening section of the physical form. The dental form is also required for kindergarten entry. A vision exam is required which is conducted by a physician who specializes in vision.

Immunization requirements for kindergarten:

1. Diphtheria, Pertussis, Tetanus: 4 or more doses of DTP/DTaP with the last dose qualifying as a booster and received on or after the 4th birthday
2. Polio: 4 or more dosed of IPV with the last dose qualifying as a booster and received on or after the 4th birthday.
3. Measles: 2 doses the first dose received on or after the 1st birthday; second dose no less than 28 days later (usually given as a MMR).
4. Mumps: 1 dose received on or after the 1st birthday (usually given as a MMR).
5. Rubella: 1 dose received on or after the 1st birthday (usually given as a MMR).
6. Hepatitis B: No requirement but if the child has received the vaccines they must be 3 doses at appropriate intervals.
7. Haemophilus Influenza B: Hib immunization not required after 5th birthday. Chicken Pox: 1 dose on or after the 1st birthday or physician’s statement verifying disease history, or laboratory evidence of Varicella immunity.

Sixth Grade students are required to have a school physical on file with the school prior to the start of school.

Fifth Grade, Seventh Grade and Eighth Grade student who wish to participate in sports are required to have a sports physical on file with the school prior to the first practice of that sport. In addition all Fifth Grade students must have the Hepatitis B series of vaccines prior to the start of their Fifth Grade year.
