Dear St. Thomas Families,   The 2012-13 school year will be my first as the St. Thomas School Nurse.  I know that God’s plan has brought me to St. Thomas, and I am thrilled to be starting on a new journey here.  I hope to be a helpful resource for any/all of your healthcare related needs.  Please know that your children’s health and wellness is of my utmost importance.

My office hours are: Monday, 7:30-2:30
Tuesday (every other), 7:30-1:00
Wednesday, 7:30-1:00
Thursday, not here
Friday, 7:30-1:00

Health Requirements Due By Start of School Year

 ***All forms must be dated so that they are valid for the current school year***

Preschool 3 yr old students: State of Illinois Physical Form, Lead Risk Assessment Questionnaire and Dental Exam Form

Preschool 4 yr old students (exempt if former Preschool 3 yr old student): State of Illinois Physical Form, Lead Risk Assessment Questionnaire and Dental Exam Form

Kindergarten students:
  • State of Illinois Physical Form, the doctor must complete the diabetes screening section of the form. 
  • A complete record of immunizations, as required by the Illinois Department of Public Health.  The only acceptable exemption will be for medical reasons as certified by an M.D. or D.O. 
  • Lead Risk Assessment Questionnaire
  • Dental Exam Form
  • State of Illinois Eye Examination Report Form.  The student must have had a current vision exam conducted by a physician who specializes in vision.
Second Grade students:
  • Dental Exam Form
Third Grade students:

No specific requirements

Fourth Grade students:

No specific requirements

Fifth Grade students:

Proof of having received Hepatitis B vaccine series (If student is participant in School Athletics, see below)

Sixth grade students:
  • State of Illinois Physical Form
  • Proof of having received Tdap vaccine
  • Dental Exam Form
Seventh grade students:

No specific requirements (If student is participant in School Athletics, see below)

Eighth grade students:

No specific requirements (If student is participant in School Athletics, see below)

Students participating in School Athletics (5th, 7th and 8th grade students):

Required to have had current sports physical on file prior to the first practice of the sport.  (This excludes 6th graders as their school physical will replace the need for having a sixth grade sports physical.)
