IMPORTANT NEWS!Flu season is here--See Flu Links Dear Parents,
Children entering Kindergarten (must be five years old by September 1st) must have a physical on file as required by Illinois State Law. By the start of the school year all students must have on file with the school a complete record of immunizations as required by the State of Illinois Department of Public Health. The only exemption accepted will be for medical reasons as certified by a M.D. or D. O. The doctor must complete the diabetes screening section of the physical form. The dental form is also required for kindergarten entry. A vision exam is required which is conducted by a physician who specializes in vision.
Immunization requirements for kindergarten:
Sixth Grade students are required to have a school physical on file with the school prior to the start of school.
Fifth Grade, Seventh Grade and Eighth Grade student who wish to participate in sports are required to have a sports physical on file with the school prior to the first practice of that sport. In addition all Fifth Grade students must have the Hepatitis B series of vaccines prior to the start of their Fifth Grade year.
